This Service describes the community Falls Prevention Service which offers diagnostic/assessment and interventional advice

Accessing the service

Referrals from GPs should be made using the OCCG GP Referral Pro Forma and emailed directly to the service

Who should be referred

Patients in the community aged 65 or over, who are ambulant, who would benefit from and are medically well enough for a comprehensive falls assessment.

Oxfordshire patients may also be referred to any of the following services to have falls assessments

  • Community Geratology Services (available primarily through secondary care)
  • Specialist Community Neurological Physiotherapy Service (if the primary issue is a neurological condition and only physiotherapy is felt necessary)
  • The Single Point of Access (if social services input is desired, or multiple agencies such as district nursing input is anticipated where falls work should be done by an integrated team)
